Skin cancer screening: Why the focus should be more on individual risk in future

Skin cancer is among the most common malignancies worldwide, and its incidence continues to rise. A recent scientific review suggests that the future of skin cancer screening may lie less in universal population-wide programs and more in personalized, risk-based approaches. While evidence for broad screening remains debated, clearly defined high-risk groups appear to benefit substantially from intensified surveillance and advanced diagnostic methods. Emerging technologies such as artificial intelligence and teledermatology may further reshape skin cancer prevention in the coming years.

A Growing Global Health Challenge

The most common skin cancers include basal cell carcinoma, squamous cell carcinoma, and melanoma. Although non-melanoma skin cancers are primarily associated with morbidity, melanoma remains the most dangerous form due to its significant metastatic potential. Rising incidence rates worldwide are creating increasing clinical and economic challenges for healthcare systems.

Early detection remains one of the most important determinants of prognosis and treatment success.

Population-Wide Screening Remains Controversial

Germany is one of the few countries offering a nationwide skin cancer screening program, providing biennial full-body skin examinations for adults aged 35 years and older. In contrast, countries such as the United Kingdom, the United States, and Switzerland rely primarily on individual risk assessment rather than population-wide screening.

Scientific evidence remains mixed. Large studies have demonstrated increased detection of early-stage melanomas, but long-term reductions in melanoma mortality have not been conclusively demonstrated. Researchers continue to debate issues such as overdiagnosis, lead-time bias, and the overall balance between benefits and harms.

Risk Stratification Is the Key

According to the review, screening effectiveness depends heavily on identifying individuals at increased risk. High-risk groups include:

  • Patients with a personal history of melanoma or non-melanoma skin cancer
  • Individuals with a family history of melanoma
  • People with numerous or atypical nevi
  • Individuals with genetic predispositions
  • Organ transplant recipients and other immunosuppressed patients
  • Persons with significant occupational UV exposure
  • Individuals with repeated severe sunburns during childhood and adolescence.

For many of these groups, screening intervals of three to twelve months may be more appropriate than standard population-based recommendations.

Genetics and Immunosuppression Matter

The review highlights the growing importance of genetic risk factors. Mutations such as those affecting the CDKN2A gene and variants of the MC1R gene have been associated with significantly increased melanoma risk, sometimes independent of UV exposure.

Immunosuppressed individuals represent another particularly vulnerable population. Following organ transplantation or long-term immunosuppressive therapy, skin cancer incidence increases substantially, often accompanied by more aggressive disease behavior. These patients require individualized surveillance strategies and closer dermatological follow-up.

Advanced Diagnostic Technologies Improve Detection

The success of screening depends not only on frequency but also on diagnostic quality. Dermoscopy is now considered the clinical standard and significantly improves diagnostic accuracy compared with visual inspection alone.

For high-risk patients, digital dermoscopy, total-body photography, and longitudinal image monitoring are increasingly being used to detect subtle changes over time and reduce unnecessary excisions of benign lesions.

Artificial Intelligence and Teledermatology: The Future of Screening

Artificial intelligence is rapidly emerging as a valuable tool in skin cancer detection. In specific tasks, AI systems already achieve diagnostic performance comparable to experienced dermatologists. Rather than replacing clinicians, these systems are expected to support clinical decision-making and improve early detection.

Teledermatology may also help address workforce shortages by enabling primary care physicians to digitally transmit images of suspicious lesions for specialist evaluation, facilitating faster triage and referral.
